This Notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Our Responsibilities

Luxury Medical Group is required by law to maintain the privacy and security of your protected health information ("PHI"), to provide you with this Notice, and to follow the privacy practices described in this Notice, as long as it is in effect.

How We May Use and Disclose Your Health Information

  • Treatment: To provide, coordinate, or manage your health care and related services. For example, we may share information with other providers involved in your care.
  • Payment: To obtain payment for services we provide to you, such as billing you directly or working with payment processors. At this time, we generally do not bill insurance on your behalf.

Certain uses and disclosures of your PHI will be made only with your written authorization, including most uses and disclosures of psychotherapy notes (if applicable), uses and disclosures for marketing purposes, and disclosures that constitute a sale of PHI. You may revoke an authorization at any time in writing, except to the extent we have already relied on it.

Your Rights Regarding Your Health Information

You have the right to:

  • Inspect and obtain a copy of your PHI in our records, subject to certain limited exceptions. We may charge a reasonable fee for copies.
  • Request amendments to your PHI if you believe it is incomplete or incorrect. We may deny your request in certain situations, but we will explain the reasons in writing.
  • Request restrictions on how we use or disclose your PHI for treatment, payment, or health care operations. We are not required to agree to all requested restrictions, but if we do agree, we will abide by them, except in emergencies.
  • Request confidential communications by asking that we contact you in a specific way (for example, at a specific phone number or mailing address).
  • Receive an accounting of certain disclosures of your PHI made by us in the six years prior to your request, excluding disclosures for treatment, payment, and operations and certain other exceptions.
  • Obtain a paper copy of this Notice at any time, even if you have agreed to receive it electronically.

Our Communications with You

We may communicate with you about your care or services using the contact information you provide, including phone, text, email, or patient portals where available. While we take reasonable steps to protect your privacy, electronic communications may carry some inherent risk. You may ask us to use alternative communication methods where feasible.

Changes to This Notice

We reserve the right to change the terms of this Notice and our privacy practices at any time, consistent with applicable law. Any new Notice will be posted on our website and available upon request, and the effective date will be updated.
